Description
During this clinical rotation, you will take care of several clients. You will select one client, and write a scholarly paper based on your observation of and experience in the care of your assigned client. Utilizing the topics listed below and evidence-based practice, you will include the following:
Introduction to the topic (select one of the topics below).
Description of your client and the care of your client.
Comparing and contrasting your textbook and evidence-based practice regarding your topic.
Conclude by summarizing and reflecting on what was learned from the assignment and how it will impact your practice in the future.

Patient Description
Include age, ethnic background (if relevant to the topic), gender, acuity level within hospital
(step down, floor, ICU), or if in a nursing home (assisted living facility, rehabilitation, or skilled
nursing), relevant medical history, diagnosis, reasons for admission, length of stay, etc. Please be
mindful of HIPPA.
Patient Care
What kind of care is the patient receiving?
How is the nursing staff currently addressing the patientàhealth concerns?
Why are the current nursing interventions important for the patientàoverall health?
How do these interventions contribute to improving the patientàhealth?
Analysis
Discuss the current standards of care in nursing related to the topic (ie., Assessment/
Scales, Goals, Interventions, Methods of Evaluation).
Discuss outcomes both positive and negative (if goals not met) for the patient. Be
sure to use examples from the literature, case studies, or direct patient care to support
your statements.
Summary
Summarize most important points from paper. Draw any final conclusions about topic.
Reflection
In this section, you may now begin to write in first person (I, me, my). Address these
questions in this section in a narrative fashion:

ow do you feel about it?
ow did it change you?
