Select one of the 77 geographic communities in Chicago and examine existing, or secondary, health data and statistics linked to the selected community. Data will be presented narratively, and/or in tables, charts maps, and disparities identified and emphasized in order to describe community need. 

The paper should be structured using the following sub-sections:

Community Demographics: include total population, ethnic/racial breakdown, and key demographics, including but not limited to education, employment, income, housing, language, and immigration status. Also include community boundaries and a map that identifies the community area.

Health Disparities: describe health disparities that the community experiences compared to the rest of Chicago. Include at least five that you believe to be important in terms of prevalence and/or severity. These disparities may be expressed by morbidity or mortality rates or behavioral health disparities.

Social Determinants: describe characteristics of the community that may be conceptualized as social determinants to explain the specific disparities previously described. Think of the indicators set forth in CDCàgroups of social determinants, and these indicators of social determinants may be expressed by some of the key demographics you have summarized previously. All of the possible social determinants may not explain specific disparities, but you must consider at least one specific determinant for each disparity you emphasize in the previous section.

The secondary data analysis constitutes the first component of the final project.
