There was an 88-year-old female who presented on 10/24/2022 for urinary tract infection (uti) symptoms and was given Ciprofloxacin for treatment. Urine culture obtained and results showed that the patientàbacteria was resistant to Levofloxacin. Patient was called to alert the change of her antibiotic from Ciprofloxacin to Macrobid. Patient came back for follow up and reported that symptoms had decreased, but she still had dysuria every now and then. Urinalysis was obtained and showed a trace of nitrates. Patient had 2 more days of Macrobid left to take. A follow up for 1 week later was made for patient.
Dr. Gupta of UpToDate states that when a female comes in with symptoms of uti such as urinary frequency, dysuria, suprapubic pain with the absence of vaginal symptoms, no additional testing is warranted to make a diagnosis. Urinalysis is useful when the patient has atypical symptoms (Gupta, 2022). Urine culture and susceptibility testing should be performed in patients with risk factors for more serious infection such as immunocompromising conditions, poorly controlled diabetes, etc. Provider may treat with best antibiotic of choice for patient until urine culture comes back to ensure patientàbacteria is not resistant to antibiotic (Chu, Lowder, 2018).
The NP should take the SHAREapproach when doing shared decision making with the patient:
- Seek the patientàparticipation.
- Help your patient explore and compare treatment options.
- Assess your patientàvalues and preferences.
- Reach a decision with your patient.
- Evaluate your patientàdecision (Agency for Healthcare Research and Quality, 2020).
Make sure that all questions and concerns the patient has are addressed. The patient will be more likely to be complaint with plan of care when they felt that they have been heard and considered.
