Developing an Action Plan

In this module, you will continue work on the health need identified in Module 1 by selecting a partnership model or collaborative strategy appropriate to your identified need and the resources required. You will use the model or strategy to plan out a collaborative change initiative addressing the need.

Create a 2-page paper (not including the title and references pages) in a Word document for your response. Use APA format.

Create a title page and references page in APA format. Your paper should have an introduction and conclusion.

Follow the steps to complete the assignment.

• Step 1. From the partnership models and collaborative strategies you researched in the Module 3 Application assignments, select a model or strategy appropriate to your identified issue and the resources required.
• Step 2. Use the model or strategy to plan out a collaborative change initiative to address the identified issue and to help you obtain the required resources.
• Develop a presentation for stakeholders, an educational program, a school or community awareness event, a public service announcement (PSA), a website, a blog, an advertisement, flyer, or brochure, etc. Whatever you decide, you should issue an appeal for resources to help resolve the issue.

Step 3. Compose a 1- to 2-page description of your plan for addressing the issue and appealing for resources. Your plan should include the scope of the issue, its impact on the school and/or community, current efforts to resolve the problem, and suggestions for potential solutions to obtain resources and resolve the problem.
