561D1 Katie: Our facility helps to keep people healthy by providing quality healthcare services for all in the community. One way this is done is by keeping good patient medical records to facilitate close monitoring of patient progress. Our facility brings together agencies from different sectors %ducation, healthcare and government 0lus grassroots organizations and others to make measurable change in communities where social determinants contribute to poor overall health (Improving Community Health, n.d.). The current focus of these efforts are on connecting people to appropriate resources that help address behavioral health and food insecurity. Our facility even offers nurses so many community hours to go out into the community and get paid for doing so as well. The Health Leads program works to build lasting partnerships and change power structures between health care, public health, community-based organizations and community members to enable community members to make decisions about the living conditions that impact their health (Health Leads, 2021). Health Leads program also works to expand, train and support community-based workforces who help provide food, housing, and other essential resources to families and communities that need it most (Health Leads, 2021). This program would work great with our facility because there are many low-income patients with long term conditions and other diseases who are caught in a vicious cycle of poverty and poor health. The presence of such cases just shows how such a program is needed by the low-income community around our facility. The program would also work well since there are a lot of college students within the area who can be trained as volunteers to help match the needy patients with the resources they need within their community. Professional medical staff are also available to provide training and directions on what patients need.

561D1chistian: I currently work in a community-based acute care hospital that treats patients who are admitted for various conditions. We also have a walk-in emergency department and accept patients from bigger hospitals through out Los Angeles. The patients we take in from other facilities typically have poor insurance or none at all, therefore not allowing them to be taken in from a hospital like Kaiser for example. My facility mostly treats the urban-poor population. After treating their acute conditions, we provide them with the education and supplies they need to be discharged home and the knowledge of what they must do to maintain their condition and avoid future hospital visits. If a patient is stable for discharge, but does not have a home or unable to take care of themselves at home, our case management will work to set up an alternative living situation such as a nursing home, rehab, assisted living, shelter placement, or even home health. I think that the Health Leads program could benefit my facility as it seems to mostly be for assisting with community-based programs. The clientele of the Health Leads program seem to be similar to the population that my facility serves. They enable community members to make decisions about the living conditions that impact their health, help provide food, housing, and other essential resources to families and communities that need it most. They also design systems that could show access to resource availability. All of this could greatly help the patients in my facility.

561D2 Kathie: Healthy people 2020 nutritional goal: Promote health and reduce chronic disease risk through the consumption of healthful diets and achievement and maintenance of healthy body weights (Libretexts, 2022).

Healthy people 2020 fitness goal: Improve health, fitness, and quality of life through daily physical activity.

Healthy people 2030 nutritional goal: focuses on helping people get the recommended amounts of healthy foods ,ike fruits, vegetables, and whole grains 4o reduce their risk for chronic diseases and improve their health (Nutrition and Healthy Eating – Healthy People 2030 | health.gov, n.d.).

Healthy people 2030 fitness goal: includes objectives focused on improving health and well-being by helping people get enough aerobic and muscle-strengthening activity(Physical Activity – Healthy People 2030 | health.gov, n.d.).

561D2 Emerencia: Healthy people (HP) deal majorly with the prevention of disease and health promotion and was initiated by the United States Department of Health and Human services (USDHHS). HP 2020 was introduced in 2010 to improve peopleàquality of life, and later onward, Hp 2030 was launched. HP 2020 and 2030 have many objectives, but the focus is on nutrition and fitness goals. HP 2020àobjective on nutrition was to encourage Americans to eat foods and to reach and maintain appropriate body weights to boost health and lower the chance of developing chronic diseases (Libretexts, 2018). To achieve this, some guidelines on healthy eating were set. First, limiting caloric, fat sodium, and sugar intake were the key. Another recommendation was to take a variety of foods rich in nutrients across all groups. Increased exercise and physical activity were also identified as one of the strategies to meet this objective. Doing all this would prevent diseases such as hyperlipidemia, diabetes, and high blood pressure. Moreover, it promotes appropriate growth and development in children. In HP 2030, the nutritional goal focused on the portion of nutritious foods required to prevent these chronic diseases. It stated that portion control is important, and people should learn how to choose healthy foods (Health.gov,2022). The strategies included increasing whole grain, vegetables, potassium, and calcium consumption, Reduction of saturated fats and sugar intake was also recommended. The strategies put in place in HP 2030 are more specific compared those to those in HP 2020.This increase the chances of people adhering to healthy living to prevent chronic disease and lifestyle -related diseases as they have more details on what they should and should not take.
