Description
Topic 1.
Using the next guide as reference create a SMART goal to improve the indicators of your health problem at short or long term:
SMART goals help improve achievement and success. A SMART goal clarifies exactly what is expected and the measures used to determine if the goal is achieved and successfully completed.
A SMART goal is:
Specific (and strategic): Goal must be clearly defined èo and what?
Measurable: The success toward meeting the goal can be measured. Outcome must demonstrate levels of change or improvement.
Attainable: Goals are reasonable and can be achieved.
Relevant (results oriented): The goals are aligned with current tasks and projects and focus in one defined area
Time framed: Goals have a clearly defined time-frame including a target or deadline date.
Examples:
Not a SMART goal:
®bsp;Reach out to stakeholders.
Does not identify a measurement or time frame, nor identify why the improvement is needed or how it will be used.
SMART goal:
(e Department will launch communications with stakeholders by conducting three focus groups specific to needs assessment and funding by the end of the first quarter.

Healthy People 2030
The most important topic covered in the healthy people website that constitutes a health
problem to my community is access to healthcare. The topic is sensitive because of the
community structure, population composition, and economic endowment that exposes the
community to healthcare access disparities. As a community consisting of a majority of
minorities in a country where racism and discrimination is a social determinant of health, access
to healthcare is a critical factor (U.S. Office of Disease Prevention and Health Promotion, 2022).
Inequality in access to healthcare is a primary factor for decreasing patient outcomes, which
contradicts a primary overarching goal of improving the quality of health and well-being for all
Americans. Health disparities in access to healthcare are decreasing maternal and infant wellbeing, increasing mental health problems in adolescents and young adults and reducing levels of
care for the senior population (U.S. Office of Disease Prevention and Health Promotion, 2022).
One criterion of understanding the problem is looking at the healthcare infrastructure, which
influences access, quality of healthcare, affordability, and health policy. Another criterion is
evaluating risk factors for secondary health problems identifiable by analyzing various social
determinants of health.
US Preventive Services Task Force (USPSTF)
One of the most significant strengths of the source is that it provides an array of preventive care
strategies through screening, preventive medication, and counseling. Moreover, it has detailed
information about how to approach various issues such as maternal and infant health, mental
health, and specifically handling alcohol and tobacco use in adolescents and young adults and
dealing with anxiety and depression (U.S. Preventive Services TASK FORCE, 2022). However,
the abovementioned issues are outliers to a bigger problem: access to healthcare. The source

does not provide concrete ways of handling the problem of healthcare access, inequality, or
social determinants of health. Therefore, considering a source modification would be adding
information about the larger complex health issues that form the umbrella for secondary health
problems (U.S. Preventive Services TASK FORCE, 2022). Lastly, it would be vital for the
source to have concise and timely solutions to various health problems rather than broad
solutions that do not incorporate the SMART framework.
