Description
Identify local, state, and national resources which facilitate safe and effective transitions of care for older adults.
Your supervisor is satisfied with the resources described in your annotated bibliography and is eager for you to continue your work. As a next step, your supervisor wants you to develop and share the following items with them.

Inservice Topic Selected-Dementia
Dementia is a syndrome in which there is deterioration in cognitive function beyond what
might be expected from the usual consequences of biological aging. Dementia is caused by
damage to brain cells. This damage interferes with the ability of brain cells to communicate with
each other. When brain cells cannot communicate normally, thinking, behavior and feelings can be
affected.
Decline in thinking skills, also known as cognitive abilities, severe enough to impair daily life
and independent function. They also affect behavior, feelings, and relationships. Dementia may
cause people to feel insecure and lose confidence in themselves and their abilities. They may feel
they are no longer in control and may not trust their own judgement. People with dementia often
experience changes in their emotional responses. They may have less control over their feelings
and how to express them. For example, someone may overreact to things, have rapid mood
changes, or feel irritable. They may also appear unusually distant or uninterested in things. People
with dementia tend to be at a high risk of falling. They are more than three times more likely to
fracture their hip when they fall, which leads to surgery and immobility.
Memory loss in a person with dementia is persistent and progressive (it continues to get
worse), not just occasional. It may affect a personàability to continue to work or carry out familiar
tasks. It may mean having difficulty finding the way home. Eventually, it may mean forgetting
how to dress or how to bathe. There are approximately 50 million people living with dementia
worldwide and this is set to increase to 75 million by 2030 and 131.5 million by the year 2050.
This increase in the number of people living with dementia is primarily thought to be due to
improving healthcare.
3
Better healthcare must lead to an increased life expectancy, therefore there is a greater
proportion of older people worldwide. The rising number of people affected by dementia and the
increasing cost have led to several countries developing national dementia strategies. These
strategies include the need for a health and social care workforce that is appropriately trained and
skilled to deliver good dementia care.

According to WHO (2022), dementia is becoming more common worldwide. Dementia is
mostly age-related, but disease or injury can develop it in younger persons. The WHOàfactsheet
on dementia is comprehensive and up-to-date. It explains dementia, its causes, and its effects
(WHO, 2022). This dementia fact sheet is from the WHO, a credible source of global health
information. This source explains dementiaàglobal prevalence and causes. It can raise
awareness of this condition and the needs of those affected.

Mukadam, N. (2020). Dementia prevention, intervention, and care: 2020 report of
the Lancet Commission. The Lancet, 396(10248), 413-446.
The sourceàmain idea is to provide recommendations for policy and practice for people
with dementia based on the latest evidence. Livingston et al. (2020) mainly suggest early
detection and intervention, risk reduction, and care and support for people with dementia and
their caregivers as the best way of dealing with the issue. The journal is credible and provides a
detailed overview of the current state of dementia care. The recommendations are sensible and
provide a good starting point for addressing the global challenge of dementia. The authors¡pproach, which is evidence-based provide a clear plan for improving the quality of care for
patients with dementia.

Canevelli et al. (2020) takes a different approach by examining how dementia patients
and care givers are affected by COVID-19. The authors identify isolation, stress, and difficulty
accessing critical services as the key issues. The authors insight that caregivers should receive
support and information to overcome these obstacles. This source is useful for practitioners
experienced workplace stressors. It is simple and offers realistic COVID-19 epidemic solutions.
However, it neglects dementia patients.eeds, which is a significant omission.
